Evaluation of the diagnostic significance of result-combinations of well-established and new diagnostic tests for vestibular disorders

Objective Besides the gold standard, caloric reflex test, vertigo diagnosis is increasingly supplemented by video head impulse test (vHIT) for all six semicircular canals of vestibular organ. Therefore, aim this study to evaluate possible gain in diagnostic significance from combination different outcome parameters same and apparative examination methods (calorics, vHIT).Methods Diagnostic was prospectively evaluated compared using target sensitivity (SEN), specificity (SPE), positive predictive value (PPV) negative (NPV) based on a collective 42 healthy 40 subjects with peripheral-vestibular pathology. Results Caloric testing achieved SEN 78% as single procedure SPE 85%. The vHIT showed 90% 86% triple asymmetry horizontal (vHITlat) anterior (gainant), if at least one three pathological. If were pathologic, an 100% achieved. For posterior vHIT, there no evidence significant difference between groups (p>0.05). A maximal NPV (vHITlat + gainant) testing. vHITlat alone established 95%. Conclusion can be preferred clinical routine investigation peripheral disorders. However, still appears superior therefore improves certainty diagnosis.
